Privacy Policy

The Customer Privacy Policy defines the basic policy of our company (*1) concerning the handling of personal information for those who receive medical treatment (those who receive medical treatment are hereinafter referred to as "customers"). The personal information indicated here refers to the personal information specified in Article 2, Paragraph 1 of the "Act on the Protection of Personal Information." (*2)

(2)Usage in business outsourcing

・Consignment of sample test work, etc.

・Consignment of office work related to insurance for medical care, nursing care, and workers' accident insurance as well as work related to publicly funded medical care

・Consignment of maintenance and management of information processing systems

・Case studies and system developments with the goal of improving the quality of medical care and relaying medical information back to society

(3)Usage to share information

・When communicating with other hospitals, clinics, pharmacies, home health care services, etc. and for the registration of image information to data centers for those purposes

・When responding to inquiries from other medical institutions

・When seeking the opinions and advice of external doctors for the medical treatment of customers

・When explaining medical conditions to family members

・When submitting receipts to a screening center, responding to inquiries from the screening center or the insurer, and usage when requesting payment for medical expenses related to other medical treatment, nursing care, and workers' compensation insurance

・When submitting receipts to government agencies related to publicly funded medical care, answering inquiries, and billing for medical expenses related to publicly funded medical care

・When notifying businesses of the results of medical checkups that were commissioned by those businesses

・When notifying or consulting with a professional organization or insurance company related to professional liability insurance, etc.

・When providing information to external auditing organizations

・Case studies and system developments with the goal of improving the quality of medical care and relaying medical information back to society

(4)Usage to distribute information

・When our company sends out information regarding our health screening centers and specialized outpatient services

(5)Usage to provide services and to improve those services

・In order to create surveys and research materials with the purpose of improving our company's services, as well as for providing services and service related information regarding our company, the group company Resorttrust, Inc. of our operational support company (Advanced Medical Care Inc.) and Resorttrust, Inc.'s subsidiaries, affiliated companies, and related medical organizations (including point services, which applies for the rest of this policy as well). Our company will not use customers' personal information outside the purposes of use listed above except in the cases below.

3.Proper management of personal information

Our company strives to keep each customer's personal information accurate and up-to-date, and to prevent the leakage, loss, destruction, falsification of and unauthorized access to their personal information. Even if the handling of personal information is outsourced to a required extent to achieve the purpose of use, we will supervise the trustee so that the personal information will be managed safely and appropriately.

4. Providing personal information to a third party

When the Medical Corporation provide the Customer's personal information to a third party, only limited data categories necessary for each utilization purpose shall be provided by means of document, film, electronic or, magnetic data, or verbal information, etc.; upon obtaining the consent of the Customer, except in cases required by the laws and regulations.

5. Confirmation and disclosure of personal information

If a customer or a proxy specified by law requests the confirmation or disclosure of said customer's personal information, our company will respond appropriately and in a timely manner based on laws, regulations and our company guidelines. Customers who would like to know how we receive and process these requests can contact us using the inquiries information provided under section 8 below.

6. Correction of personal information

If a customer or a proxy specified by law requests the correction, addition, erasing, usage suspension, deletion, or suspension of provision of personal information to a third party because the content is incorrect, we will promptly start an investigation into the reasons for the request and respond appropriately based on laws and regulations. Customers who would like to know how we receive and process these requests can contact us using the inquiries information provided under section 8 below.

7.Joint usage

(1) Our company will share personal information that contains medical information with the organizations below (hereinafter referred to as "joint usage organizations") under our company's management in line with the "2. Purpose of use of personal information" section above.

・ Personal information being shared

Name, date of birth, age, gender, address, email address, telephone number, patient's ID, medical institution, medical record data (injuries and diseases, dates of consultation, medical record contents), and health screening data (images, values, findings, consultation details, guidance, and result summaries for each screening)

・Joint use organizations

Himedic Clinic WEST, Midtown Clinic Medical Corporation, Trust Clinic, Keisokai Medical Corporation, The University of Tokyo Hospital, Kyoto University Hospital, and other affiliated medical institutions of the Grand Himedic Club, Shinkokai We will send notifications about additions and name changes to the organizations above through this website, when patients visit, and via post.

(2) Our company will jointly use personal information (except the patient ID containing medical information, medical institution, medical records, and screening data listed under (1) above are not included) with the organizations listed below and other companies (hereinafter referred to as "joint use companies") under our company's management.

・Purpose of usage at joint use companies

For the provision and operation of point services by joint use companies and the provision of their service-related information, and in order to create surveys and research materials with the purpose of improving these companies' services

・ Personal information being shared

Personal information that is related to the provision or operation of point services, including name, date of birth, age, gender, address, email address, telephone number, membership status, purchase history, history of facilities used, point total, point expiration dates, and point usage history
