Information regarding telephone consultations

In our efforts to prevent the spread of COVID-19, our clinic is offering telephone consultations for all internal medicine appointments.

◆The general flow of a phone consultation◆

①Schedule an appointment

Please schedule an appointment by accessing the Online Reservation system below.
*Please select the internal medicine (telephone consultation) option.

②Complete the online medical questionnaire

There are several different online medical questionnaires. Please complete the appropriate one for your appointment.
The following information is required to create your medical chart and to process payments. Please be careful not to miss anything when completing the questionnaire.

Required Information:
1.Name
2.Date of birth
3.Address
4.Phone number
5.Your e-mail address
6.Attached copy of your health insurance card
7.Attached copy of your medication record (if you have one)
8.Indicate how you wish to receive your medication (either by mail or by pick up at the pharmacy of your choice)

*If you would like to pick up your medication at the pharmacy of your choice, please make a note of the name (branch name included), phone number, and fax number of the pharmacy under the section “Other requests upon medical treatment” of the questionnaire

③Telephone consultation

The physician will call you when it is close to your appointment time. Please be prepared to take the call.
We ask that you set aside 15 minutes before and after your appointment time, as the start time can vary depending on the appointments before you.

④Receiving your prescription

You have the option to (A) have it mailed or (B) pick it up at the pharmacy of your choice.

A: Have it mailed
Your prescription will be sent by postal mail from the nearest pharmacy. The prescription fee will be paid upon delivery along with a shipping and cost-on-delivery fee of 530 yen. Please note that in order to mail you the prescription, we will need to provide the pharmacy with personal information such as your name, contact information, address, prescription, etc.
B: Pick it up at the pharmacy of your choice
After completing the telephone consultation, the clinic will provide the prescription order to the pharmacy of your choice. Please contact your preferred pharmacy directly to see if your medication is ready for pick up. Please pick up your prescription within 4 days, including the prescription date.

*Please be sure to specify the name (including branch name), phone number, and fax number of your preferred pharmacy when completing the Online Medical Questionnaire.

⑤Paying the medical bill (E-payment system, bank transfer or payment at the reception desk)

Payment via E-payment system (Paysys)

Following your consultation, you will receive a payment URL link via SMS or e-mail. You have the option to pay either by credit card or through the convenience store payment system.
※Please note that you will be charged a service fee of 275 yen.

Payment via bank transfer

Following your consultation, you will receive the payment and transfer account information via e-mail from the address below. Please make the payment within 2 weeks of receiving the e-mail.


*Please be sure to enter your e-mail address when filling out the Online Medical Questionnaire.

*If you have spam protection set, please make sure to change the setting so that you can receive emails from @amcare.co.jp.

*You are responsible for any bank transfer fees.

Payment at the reception desk
You can also pay at the clinic's reception desk. Please come to our clinic during business hours, and make the payment within 2 weeks of the consultation.

Precautions and notices

  • ・Please note that this a temporary measure placed in the light of the COVID-19 pandemic. This flow is subject to change due to revisions by the Ministry of Health, Labor and Welfare, and shifts in infection trends.
  • ・Please note that you are not eligible for a telephone consultation if face-to-face consultations or diagnostic tests are required for your medication or symptoms.
